How to Apply

The application process starts with a phone call to Southwest Service Dogs at 479.754.3682. A Program Application form and a Medical History Form (to be filled out by the applicant's physician) need to be filled out and returned prior to an actual interview.

Don't let these forms scare you off. We need to get an idea of the kind of assistance you require and an idea of your personality type. At Southwest Service Dogs we believe that a good "personality" match between Service Dog and Client are of the utmost importance. After all, if a good match is made, you should be partnered with your dog for many years.

The wait between an application being received and the actual placement will vary because of the importance of the "match." If we have a dog that is ready to graduate that is a match for your personality type, the wait could be very short. Of course, if we don't have an immediate match, the wait might be longer.
